About the Patient Search Module

The Horizon Patient Folder database stores patient and encounter (episode of care; visit) information that is typically introduced from the hospital information system (HIS). The Patient Search module allows you to locate patients in the Horizon Patient Folder database using a variety of search criteria. When you have located the patient and encounter you need, you can view document images and other information associated with the patient.

The Patient Search window may be your default module window when you log in or you can select it by clicking the Patient Search tab.

Your searches may be for patients in one facility. Optionally, for multi-facility enterprises, you can search in one or all facilities. Only the facilities to which you have access rights are listed.

An assumption for using the Patient Search module is that your patient access rights have been set in the Horizon Patient Folder Workstation.

The Patient Search module allows you to use criteria associated with patient-level information to perform searches. This includes a variety of demographic data such as name, age, and Social Security Number, as well as the Medical Record Number and Global Person Identifier. Other fields, such as age, are optional and can be used to refine the search.

The Patient Search module also allows you to use criteria associated with encounter-level information to perform searches. This includes the actual encounter number (account) for an episode of care, as well as the admission and/or discharge date.

Whenever possible, you should enter as much information as you can to identify the patient. This might be a specific MRN, or a combination of fields, such as last name along with the age or date of birth. Searching in this way helps filter the data, which provides a faster search with more accurate results.

For those patients whose specific data may be unavailable, the Patient Search module allows you to search using partial data. You can enter as few as three characters (or numbers) in the following fields:

Important: When searching with partial data, ensure that your search is as specific as possible. For example, if you have a large database and search on the last name SMI, or the encounter number 000 (if all your encounters are in the format 000xxxxxx), the search may be so broad that it returns a large number of records. The search results display is limited to 500 results and the system alerts you with a message.
